Physician Directories
Folio Physician Directories are divided into a complete Physician
Section and a Healthcare Facility Section.
The Directories are published annually. Each directory is completely
revised every year with direct verification of each physician and
healthcare facility.
The physician information includes multiple practice addresses;
mailing addresses (if they differ from practice address); phone,
fax and emails; phone extensions and 800 numbers; identifiers such
as UPINs and NPIs; medical specialties with board certification;
medical school and year of graduation; hospital affiliations; HMO
affiliations; and websites.
The facility sections of the Directory include all hospitals and
nursing homes in each state as well as a complete listing of group
practice and ambulatory care centers with their individual physician
rosters.
Folio’s physician and healthcare facility information is developed
from a combination of public data sources, proprietary software
and its own direct data gathering and verification processes. Folio
is unique in the medical industry in developing a state-wide census
of all practicing physicians and in directly verifying practice
information each year with every provider. Much of the proprietary
information in Folio databases results from updates made directly
by physicians and facilities using Folio’s website, www.FolioMed.com.

Physician Database on CD-ROM
Folio Marketing CD-ROMs contain all the information in the Folio
Printed Directories, but include a powerful query tool that enables
the user to select and view records based on a single variable or
a combination of variables. Querying can be done based on a physician’s
name, practice name, type of medical practice, address, specialty,
distance around a town or zip, or a combination of these variables.
Each Marketing CD comes with an export capability that allows the
user to export up to 2,000 records
(standard Marketing CD, extra downloads can be purchased in increments
of 1,000) in a variety of formats, including, spreadsheet, delimited
text or dbf and to export any or all fields in the record.
