Update Your Listing (DDS,DC,OD,DPM,DDS)

Every day Folio surveys public and other sources of information for changes in the healthcare community. But by far, the most important contributions to the Folio database are those updates we get directly from healthcare professionals. We want to have your facility's correct and up-to-date address, phone and fax numbers, website and email address, and physician roster so other professionals can locate you for a patient referral or to complete a medical record transaction.

Please Note: The updates you make here will not be added to our database until verified.
In an effort to provide additional security to those updating via the website, Folio now requires that all facility updates must be done using the Facility ID.

Your Facility ID is a unique Folio identification number that was automatically assigned to your facility when first entered into the database. Your Facility ID is located at the upper right hand corner of the facility proof form that was mailed to you. You may also request your Facility ID via email by clicking here or call us at 1-800-223-2233 and we will be happy to provide you with this information. For physician updates please use the Physician Update menu option.
