But by far, the most important contributions to the Folio database are those
updates we get directly from listed physicians.
We want to have your current professional work addresses, medical specialties, and practice settings so other professionals can locate you for a patient referral or to complete a medical record transaction. Please Note: The updates you make here will not be added to our database until verified. In an effort to provide additional security to those updating via the website, Folio now requires that all physician updates must be done using the physician MDID. Your MDID is a unique Folio identification number that was automatically assigned to you when you were first entered into the database. Your MDID is located at the upper right hand corner of the physician proof form that was mailed to you. You may also request your MDID via email by clicking here or call us at 1-800-223-2233 and we will be happy to provide you with this information.
